All three CHL locations — in West Lafayette, Hammond and Fort Wayne— are hosting open houses. Benefits-eligible employees and their dependents covered on a Purdue health plan are encouraged to stop by the CHL open house at the location most convenient to them.
Open houses take place as follows:
The open houses provide a first-hand opportunity to become more familiar with how the CHL can help individuals with their overall health and wellness; meet their providers and health coaches; and learn about the services offered, including:
- Primary care
- Weight management
- Health care navigation
- Medication Therapy Management
- Telehealth
